Minnesota School Immunization Law

Minnesota Statutes, Section 121A.15, requires all students enrolling in an elementary or secondary school to show evidence of immunization or properly documented exemption.

The Stewartville School District requires a printout from your health care provider as proof of immunizations. No parent phone calls or handwritten notes will be accepted.

If your child will be entering Kindergarten or 7th grade in the Fall, please complete the required immunizations and turn in the proper documentation before the first day of school. Letters will be sent out in the Spring and late Summer to those students who are not in yet compliance with immunization documentation.  Stewartville School District requires a printout from your health care provider as proof of immunizations.  No parent phone calls or handwritten notes will be accepted.

Exemption From Immunizations


Medical Immunization Exemption:

If immunization is medically contraindicated for a child, or if there is laboratory confirmation that a child is already immune to certain diseases against which immunization is normally required, the student may submit a statement to this effect, signed by a healthcare provider, in order to be considered exempt from the contraindicated or unnecessary immunization.

Elective Immunization Exemption:

If a child’s parent or guardian, or the child (in the case of an emancipated minor), wishes to be exempt, based on their beliefs, from one or more immunization requirements, the parent or child may submit a statement to this effect, signed by the submitting person and notarized by a notary public.

A notarized immunization exemption can be completed on the backside (2nd side) of the Student Immunization Form
